"Issue/Transfer Request" (Revised 08-12) - If you have an item you are giving to or receiving from another department or sending to Surplus (both inventoried or non-inventoried), you will need to complete an "Issue/Transfer Request" Form.

"Missing or Stolen Property Report" Form(Revised 11-08-11) The purpose of this form is to notify the TAMIU Property Manager of missing or stolen equipment. It is to be filled out as soon as item in inventory is discovered to be missing or stolen. The form should be signed by the department head and submitted to the TAMIU Property Manager attached with correspondence from the department head documenting that step 1-9 listed on the form were taken and a copy of the police report if stolen.

The state defines any item as missing as any personal property that has not been physically located. Each department must notify the Property Manager immediately of the missing status of property upon recognition. THe state requires a two year waiting period before the equipment can be removed from the department's physical inventory. During these two years, the equipment will still show up on the physical inventory. During these two years, the equipment will show up on the physical inventory, but will have the word "Missing" under location. The department should also search for the missing item during the two years waiting period. After the two years have been completed, the item will be deleted by the Property Manager.

The state defines any item as stolen as any personal property missing by known theft, whether by forced removal, burglary, theft by employees, or other criminal acts. Stolen property must be reported to the University Police Department with 48 hours of identifying a theft of personal property.  The item will then be removed from the department's inventory as stolen.

"Issue Out Off-Campus" Form (Revised 03-04-08) - This form can be use when any item that is inventoried will be use of campus area. It is the responsibility of the department head to assure that the item issued out will be return back in good condition when no longer in use. Item issue out will be use for the sole purpose of university related work.

"TAMIU Property Deletion" Form (Revised 12-13-11)) - The purpose of this form is to notify the TAMIU Property Manager of equipment that has been sold, cannibalized, or traded-in. The state defines cannibalization as the authorized removal of components from one item of property for installation on another item of property to meet a specific requirement and/or to return an item to service.

It is to be filled out as soon as item in inventory has changed to one of these three statuses.  The form should be signed by the department head and submitted to the TAMIU Property Manager attached with any supporting documentation discussing the transaction that results in equipment disposal.  This form should only be used when equipment is sold, cannibalized, or traded-in.  Equipment that has been deemed to be either missing or stolen should have a corresponding "Missing or Stolen Property Report" Form filled out for each qualified piece of equipment.

"Department Head/Alternate Designation" Form (Revised 09-06-06) - The purpose of this form is to keep record of any changes made in the status of any Department Head.  This form is also used to assign inventory stewardship to a person designated by the current Department Head as their Alternate Department Head.  The use of assigning a steward can be helpful for departments where the daily management of inventory at the Department Head level is not practical or feasible.  The form should be signed by the Department Head and/or the Alternate Department Head then submitted to the TAMIU Property Manager when a change in either designation takes place.
